It is with great sadness that we announce the death of Avery Teresa Moore (Augusta, Georgia), who passed away on July 1, 2018, leaving to mourn family and friends. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.

She was predeceased by: her grandparents, Teresa Bohannon and Dwayne Moore. She is survived by: her parents, Alison Nicole Moore and Steven Dwayne Moore. She is also survived by grandparents, aunts, uncles, and cousins.

In lieu of flowers, we are requesting donations be made to the Avery Moore Foundation which will be set up in her honor in hopes of helping other children and families devastated by the reality of congenital heart disease.
