He is survived by: his wife Jan Stephens; his children, Rebecca (Tommy Hewitt), Nathan (Micah Slack) and Jamie (Matt Gunnels); his grandchildren, CJ Slack, Isabella Slack, Lilly Hewitt and Hadley Hewitt; and his siblings-in-law, Jay Smith and Loretta Smith.
