He is survived by: his wife Pam Maddox; his sons, David Maddox Jr., Gage Maddox and Grant Maddox; his stepchildren, Cameron Barnes (Kara), Dustin Maddox and Adam Nowling; and his grandchildren, Clayton and Canon Barnes.
