He was predeceased by: his parents, Otto Leavens and Thelma Roberts Leavens; his wife Betty Daniels Leavens; and his son Johnny Wayne Leavens. He is survived by: his daughters, Tammy Louise Leavens (Junior Gilbreath), Fiancee and Sherry Surrency of Adel, GA; his grandchild Jamie Nicole Harper (Rusty) of Broxton, GA.; his children, Jim Barkley Gilbreath of Douglas, GA and Serina Shyanne Tomberlin (Zyan) of Douglas, GA; and his great grandchild Number. He is also survived by nieces and nephews, cousins and many friends.
