He is survived by: his wife Christine Hornback; his sons, Caleb Hornback and Colin Hornback; his sister Dawn Cushard (Jay); and his mothers-in-law, Joyce Westfall (Richard), Anna Brown (James), Eva Lewis (John) and Patty Dunigan (Mike). He is also survived by many nieces and nephews.
