He is survived by: his wife Judy Chadwick; his children, Scott Chadwick and Tammy Swafford (Ronnie); his sister Mildred Dunn; his grandchildren, Samantha Chadwick, Scotty Chadwick, Shannon Chadwick, Rachel Mallett (Arron), Rhonda Johnson (Jonathan) and Regan Teem (Nathen); and his great grandchildren, Kyle Jackson, Kaden Jackson, Annika Jackson and Jeremiah Johnson.
