He was predeceased by: his parents, William Cook and Lillie Cook; and his siblings, Carter Cook, Eugene Cook and Marie Cook.

He is survived by: his wife Betty Foster Cook of Griffin; his daughter Beth Ely of McDonough; his son-in-law Greg Ely of McDonough; his grandchildren, Phillip Ely of Macon and Reed Ely of Statesboro; his sister Betty Rooks of Gainesville; and his brother-in-law Harvey Rooks of Gainesville. He is also survived by numerous nieces, nephews and cousins.
