He was predeceased by: his wife Martha Ann Fountain Boltz; his parents, Earl Boltz and Hilda Boltz; and his siblings, Dorothy Hughes and Robert Boltz.

He is survived by: his brothers, Larry Boltz of Toledo, Ohio, Rev. Joe (Shirley Fountain), Jimmy (Bernice Fountain), Junior (Jean Carver) and Paul (Frances Millsap). He is also survived by Nieces, Nephews, Great-Nieces and Great-Nephews.

In lieu of flowers, donations can be made to the American Heart Association.
