He was predeceased by: his father Charlie Thomas Cook, Sr.; his wife Deborah M. Coppedge Cooke; his daughters, Dalena and Danyae Cooke; his nephew Devin Payton; and his brother-in-law Herbert Stephens.
He is survived by: his mother Bencie M. Cook; his son Charles M. Cooke "Chuck"; his sister Deborah Cook; his sisters-in-law, Jacqueline Stephens and Gwendolyn Payton; and his nieces and nephews, Brandi Burdette, Heather Prather "Dailo", Brian Stephens, Herbert Stephens, Jr., Justin Stephens, Lawrence Merritt and Stephen Payton. He is also survived by other nieces, nephews, relatives and friends.
