He was predeceased by: his parents, Raymond Cook and Sarah Alley Cook. He is survived by: his wife Joyce Headden; his children, Sandie Blair (Stephen), Tommie Pegg (Kevin) and Gary Headden; his grandchildren, Scottie Woodfin, Ryan Headden, Kyler Blair and Kemper Blair; his sister Brenda Robinson; his niece Pamela M Flowers (Greg); and his great-nephew Parker Griffin.
