He was predeceased by: his parents, John Wright "Jake" Smith, II and Mildred Aileen Snow Smith; his parents-in-law, Jennie Bell and Roy Medlin; his wife Wanda Jean Medlin Smith; his son-in-law Jonathan Brookins; his siblings, Billy Smith and Patsy Smith; and his brothers-in-law, Andy Medlin, Ed Ketchum, Harold Smith, Willie Dunn and Edmond Medlin.
He is survived by: his children, Amy Flury (Sam) of Milledgeville, Michelle Brookins of Milledgeville, John W. "Jay" Smith, IV (Angie) and Roy Shannon (Tina); his grandchildren, Tiffany Johnson, Hunter Johnson, Jaylin Smith (Blake), Corey Brookins (Shelby) of Ashton, Katie Lumley (Cody), April Screws (Michael) and Ashley Flury; his siblings, Marian Medlin of Milledgeville, Janie Dunn of Milledgeville, Marti Pozzi (John) of Alaska, Roger Smith (Shawn) of Wyoming, Jimmy "Shorty" Smith (Dawn) of Eatonton and Richard Smith (Beverly) of Eatonton; and his sisters-in-law, Leone Smith, Bonnie Ketchum, Sue Kitchens (Tommy) and Sandy Lee. He is also survived by 5 great grandchildren, numerous nieces and nephews.
Visitation will be held on Thursday, December 18th 2025 from 5:00 PM to 7:00 PM at the Freedom Church (500 Underwood Rd, Milledgeville, GA 31061-4176). A funeral service will be held on Friday, December 19th 2025 at 2:00 PM at the same location.
In lieu of flowers, the family has requested that. Memorial Donations be made to Freedom Church, 500 Underwood Road, Milledgeville, GA 31061.
