He was predeceased by: his grandmother Geneva Lewis. He is survived by: his parents, Jack Daniel Lewis, Sr. of Rocky Face and Deloris Kay Pettigrew Short of Rocky Face; and his grandparents, Wayne, Diane Pettigrew of Lafayette and James Lewis of Rocky Face.
