He was predeceased by: his father-in-law Gary Jacobs. He is survived by: his wife Mary Sisk; his children, Joseph and Georgia Sisk; his parents, Robert A. Sisk and Anna Sisk; his siblings, Laura Luquire (Fred), Lydia Williams (Russ) and Robert Sisk (Trisha); his brother-in-law Edward Jacobs (Stacey); his mother-in-law Bonnie Judice Jacobs; and his aunt Becky Judice. He is also survived by several nieces, nephews, and cousins.

In lieu of flowers the family requests those who wish to express sympathy to consider making a donation to the Bereavement Fund of David Sisk's children.
