He was predeceased by: his mother Susan Belvin. He is survived by: his wife Jennifer Belvin; his children, Fallon, Sebastian, Rae, Erick, Elton and Perce; his grandchild Freya; his father Kim Belvin; and his brothers, Tim Forrester, Kent Belvin and Aryk Christian.

In lieu of flowers, the family respectfully suggests memorial donations be given to a fund benefiting Adam Belvin.
