He was predeceased by: his parents, Helen Ullestad and Elmer Ullestad; and his brothers, Harold and Don. He is survived by: his significant other Mary Lou; his sister Eleanor; and his children, David (Lynn), Randy (Patricia) and Deeann Forster. He is also survived by his many grandchildren and great-grandchildren.
