He was predeceased by: his grandparents, Joe Middlesworth, Lola Van Middlesworth, Ken Ludwig and Ruth Ludwig. He is survived by: his parents, Gary Middlesworth of Cedar Rapids and Carol Van Middlesworth of Cedar Rapids; his brother Jared (Beth); his nieces, Kate, Abbey, Bonnie (Cliff) and Meyer; his grandmother Janet Van Middlesworth; his aunt Sandy Van Middlesworth; and also, Van Middlesworth, Bettendorf, Chad, Keili, Lari, Dani Corely and Greg Clift.
Memorial donations may be directed to University of Iowa Children's Hospital.
