He was predeceased by: his brother Edward; and his parents, Florian Dvorak and Mary Dvorak (Severe). He is survived by: his daughter Tracy Hall; his grandsons, James Hall, William Hall and Timothy Hall; his siblings, Roger (Gayla) of Cedar Rapids, Jennie Seeck (Dale), Dean (Ellen) of Arizona and Dianne of California; his sister-in-law Vicki Dvorak of Mississippi; and his wife Sheila Allen.
