He was predeceased by: his parents, Joseph Havlik and Anna Havlik (Hillmer); and his siblings, Glenn Havlik, LaVerne "Tub" Havlik, Gladys Sturm and Helen Black.
He is survived by: his wife Alicia Augusto; his sons, Joseph J. Havlik (Sheree) of Dallas and Ric Havlik (Doreen) of Broken Arrow, Oklahoma; and his grandchildren, Joe R. Havlik (Tracy), Shannyn Chance (Chris), Jennifer Garcia (Robert), Chantel Berger (Jeff), Mike Havlik and Jay Havlik (Candance). He is also survived by eleven great-grandchildren; one great-great-grandson.
