He was predeceased by: his parents, Lester Horak and Lillian Horak (Pelisek); and his son Daniel. He is survived by: his wife Janice; his son Nicholas; his brother Don Horak (Barb); his brother-in-law David Johnson (Kay); his sibling's children, Jeffrey Johnson, James Horak and Lara; and also, French and Diekema.

Memorials may be directed to Bur Oak Land Trust, 5 Sturgis Corner Drive, Iowa City, IA 52246, or to a charity of the donor's choice.
