He was predeceased by : his parents, Wilbur Cook and Marguerite Bradley Cook; his son Merle Cook; and his brothers, Don and Bill Cook.

He is survived by : his daughter Jodie Borts (Lonnie) of Colfax, IA; his grandchildren, Jessica Borts, Terra Webb (Chris) and Dustin Cook (Wendy); and his friends, Jim Lane and Terry Bonnett.
