She was predeceased by : her parents, John and Rose Jakoubek (Paca); her husband Leonard Formanek; her son Joseph Formanek; her great grandchild Lenny Formanek; her siblings, Arlene Wellik (Paul) and Robert Jakoubek; and her siblings-in-law, Daniel Formanek (Mary) and Dorothy Nedved (Leo).
She is survived by : her sons, David Formanek of Garner, Bernie Formanek (Lorrie) of Garner, Mark Formanek of Albany, MN and Keith Formanek (Cari) of Britt; her grandchildren, Brian Formanek (Ashley), Kendra Haugen (Steve), Eric Formanek (Becca), Michele Frost (Justin), Joseph Formanek (Hope), Marie Klassen (John), Jesse Formanek, Nick Schmidt (Angie), Taylor Sanger (Mark) and Cory Formanek; her sisters, Gert Wellik (Leo) of Garner and Margaret Young (Paul) of Garner; and her sisters-in-law, Mary Jane Gaffey of Charles City and Darlene Nielsen (Hans) of Fort Dodge. She is also survived by 16 great grandchildren, many nieces and nephews.
