She was predeceased by: her father Ralph Sladek; and her siblings, Bea and Don. She is survived by: her mother Phyllis Sladek; her sister Kay Evans; her nephews, Brian Evans (Jen Evans) and Cheyanne Evans; and her close friends, Jacque Collett and Joseph Green. She is also survived by many friends and cousins.

In lieu of flowers, donations may be made to help with funeral expenses or given to Camp Courageous on behalf of Debbie.
