He was predeceased by: his parents, Leo Cook and Katherine Cook. He is survived by: his wife Donna Frerk; his children, Tim of Cedar Rapids, IA and Carla Miller (Randy) of Waterloo, IA; and his grandchildren, Taylor of Waterloo, IA and Andrew of Iowa City, IA. He is also survived by many other nieces and nephews.
As mandated by proclamation, social distancing must be practiced.
