He was predeceased by : his parents, George Slade and Edith Slade (Teague); his siblings, Kenny, Russell, Raymond, Billy and Donald "Tucker"; and his nephew Steve.

He is survived by : his siblings, Richard Slade, Bob Slade (Sandy), Mike Slade and Shirley Slade; and his sisters-in-law, Delores Slade and Anita Slade.

In lieu of flowers a memorial fund has been established in David's memory.
