He was predeceased by: his great-grandparents, Andrew Armstrong, Shirley Flaherty, Cornelius, Johanna Toering, John and Margaret Bolkema.
He is survived by: his parents, Ryan Bolkema and Erin Flaherty Bolkema of Oskaloosa; his brother Parker Bolkema; his grandparents, Lynn Armstrong of Oskaloosa, Kurt Flaherty (Kelly) of Georgia, Loren Bolkema (Lois) of Oskaloosa and Sue Toering of Minnesota; his great-grandparents, Pat Flaherty and Ardith Armstrong; and his cousins, Andrew Flaherty (Jodi), Bridget Flaherty of California, Meaghan Flaherty of Iowa, Patrick Flaherty of Iowa, Jodie Van Essen (Scott), John Bolkema (Jess) and Heidi Bolkema of Nebraska.
