He was predeceased by: his father Russell Slade. He is survived by: his mother Dolores Slade (Kofron) of Coralville; his siblings, Cyndi Langel (Rod) and Tim Slade (Peggy); his children, Ryan Slade (Amanda) of Coralville, Morgan Driscoll (Jordan) of Hiawatha and Jessica Kyer of Dayton, Ohio; and his grandchildren, Makayla, Elizabeth, Olivia Slade, Brody Driscoll and Penelope Kyer.
