We are sad to announce that on January 7, 2021, at the age of 84, Donna (Diane) Formaro of West Des Moines, Iowa, born in Des Moines, Iowa passed away. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.

She was predeceased by : her parents, Junior Clemens and Marian Clemens (Wright); her husband Phillip; and her siblings, Marlene and Bill.

She is survived by : her grandchildren, Makayla, Dawn, Schauna and Michael; her daughters, Michelle Chambers (Mike) and Teresa Terry (Shawn); and her sister Marilyn Hall-Cook (Bruce). She is also survived by several nieces and nephews.

Visitation was held on Saturday, January 16th 2021 from 2:00 PM to 5:00 PM at the Iles Grandview Park Chapel (3211 Hubbell Ave, Des Moines, IA 50317).
