He was predeceased by: his parents, Theresa Mae Huntsman and James Willard Cook of Las Vegas, NV.. He is survived by: his children, Camille Campbell (Jerel) of Mesa, AZ and Korbin Cook (Ashley) of Idaho Falls, ID; and his girlfriend Kaye Kiryu of Winnetka, CA.. He is also survived by seven grand-kids.
