He was predeceased by: his parents, William James Forker Jr. and Ruth Viola Moore Forker; his brothers, James Moore Forker, Airedales, Bailey and Loki; and his pet Yogi.
He is survived by: his wife Carol Louise Annable Forker; his daughter Morgen Brianna Forker-Lopath; his son-in-law Daniel Henry Lopath; his siblings-in-law, Dr. William Annable and Cathy Annable; his cousins, Ginny Moore, Airedale Archie and Doberman; and his pet Sasha.
