He was predeceased by: his parents, John G. Quellmalz, Sr. and Ada Quellmalz (Wedel); his wife Shirley Mae Quellmalz (Brock); and his granddaughter Kelsey Dudici. He is survived by: his daughters, Debra Chamberlin and Sherri Louth (Chris); his grandchildren, Deanna Bushkin (Brett), Kristin Schneider (Sean) and Colten Dudici; his great grandchildren, Connor Bushkin, Harper Schneider and Hadley Schneider; and his nephews, Frankie Glenn Franklin (Barb) and Frank Brock.

Memorials may be made in John's memory to Breast Cancer Research Foundation.
