Donald C. Sladek Obituary

With heavy hearts, we announce the death of Donald C. Sladek of Chicago, Illinois, who passed away on October 15, 2025 at the age of 75. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.

He was predeceased by: his parents, Otto Sladek and Lillian Sladek; his siblings, Robert Sladek and Anna Pawlicz (Ken); his parents-in-law, John and Rosalie Rusin; and his sisters-in-law, Joan Bakakos (late Phil) and Mary Ann Yanke (late William).

He is survived by: his wife Janet; his sons, Kevin (Sabrina Boyle) and Steven Sladek (Jaclyn); his grandchild Scottie; his sisters-in-law, Angeline Laba (late Bill) and RoseAnn Sochacki (late Edwin); and his sibling's children, Beth, Jenny, Angel, Edwin, Jonathan, Thaddeus, Daniel, Noah, Jonah and Theo.

Visitation was held on Sunday, October 19th 2025 from 2:00 PM to 7:00 PM at the Richard J Modell Funeral Home & Cremation Services (12641 W 143rd St, Homer Glen, IL 60491). A funeral service was held on Monday, October 20th 2025 at 8:45 AM at the same location. A funeral Mass was held on Monday, October 20th 2025 at 10:00 AM at 6949 W 63rd Pl, Chicago, IL 60655. A burial was held on Monday, October 20th 2025 at 11:30 AM at the Resurrection Cemetery (7200 Archer Rd, Justice, IL 60458).
