He is survived by: his wife Kim; his daughters, Rachel and Leah Jacoby; his mother-in-law Lillian; his brother Jeffrey Jacoby; his siblings-in-law, Heidi Forte (Matthew) and Jamie Wainick (Michael); his daughter Chloe; and his pet Chloe.
