He was predeceased by: his parents, Edward Perry and Norma Perry (Harms); and his brother Gary Perry. He is survived by: his wife Shelly Dusten; his children, Stephanie Hefke (Clint) of Marengo, IN, Chad Collins of Brimfield, IL and Chris Hofmann (Debra) of Peoria, IL; his grandchildren, Cody, Brock Hefke, Theo, Nolan Collins, Alyssa, Grace and Kara Hofmann; and his siblings, Pam Nutter (Dave) of Pekin, IL and Randy. He is also survived by his nieces and nephews.
