She was predeceased by : her mother Frances Bonefas Klein; her stepmother Lavonne Klein; and her granddaughter Melany Kozak. She is survived by : her father Wayne Klein of LeRoy; her spouse Russell L. Kozak; her children, Joshua Kozak (Ginger) of Ottumwa, Amie Schmitz of New London, Michael Kozak (Miranda Zobeck) of New London, Christopher Kozak (Kirsten) of Chicago and Amanda Kozak (Joe Eichholz) of Colorado Springs; and her siblings, Michael Klein of Elma, Mark Klein of Riceville, Joel Klein of Minneapolis, Jeanne Kulow of McIntire, Judy Green of Mason City, Jeff Klein of New Haven, Jan Klein of Dallas Texas and Michelle Cummings of Riceville, Iowa.. She is also survived by thirteen grandchildren.
