He was predeceased by: his parents, John Surlak and Apolonia Surlak; his siblings, Veronica DeKeyser (John), Caroline Cerney (Joe), Annie Ptacin (Iggy), Milly Johnson (Arnie), Pauline Supol (Tony) and Emil Surlak (Marion); and his friend Johnny Klein.
He is survived by: his children, Jodee Cizek, Cindy Camarena, Mick Surlak (Bobby Reid), Papa of Josh, Jaime (James), Chris, Zach, Brandon and Jenna; and his sibling-in-law Pete Niemiec (Janie).
