He was predeceased by: his parents, Clarence Leslie Cook (Tom) and Martha Irene Kitchen. He is survived by: his daughters, Megen Cook, Rose Cook, Michelle Hoffman, Laura Cook and Amy Cook; and his sister Sandy (Dennis). He is also survived by grandchildren and great-grandchildren.
