He was predeceased by: his parents, Roy Cook and Delores Cook (Kinser); and his siblings, David Cook, JD Elmore, Emeril "Red" Elmore, Ann and Jane.

He is survived by: his brothers, Terry Cook of Enid and Leroy "Pete" Cook of Louisiana; his daughters, Patricia Belsik of Seattle, Christina Ashcroft of Bushnell, Trudy VonSeebury of Georgetown and Candace Cook of Abingdon, Illinois; and his grandchildren, Amber Winkles of Cedar Park, Katrina of Bushnell, Jessica Fidler of Bushnell and Keenan Hall of Abingdon. He is also survived by four great grandchildren.
