She was predeceased by: her mother Sheila Henry; and her grandparents, Orville Slifer and Ruth Slifer. She is survived by: her siblings, Jason Moore (Heidi), Justin Moore, Jeremy Moore (Melissa) and Heather Nelson (Evan); her aunt Beverley Urschel (David); her nieces and nephews, Marissa, Jordyn, Rori Moore, Michael Lamping (Tara), Michelle Dixon (Caleb), Megan, Travis, Tyler, Mason, Tyson, AnnE, Logan, Aria, Kalel and Winter Moore; her children, Phoebe and Lola; and also, Wesley Urschel and Marc Urschel. She is also survived by nieces and nephews.
In lieu of flowers, memorials in her name to the Almost Home Foundation, P.O. Box 308, Elk Grove Village, IL 60009-0308 would be appreciated.
