We bid farewell to Philip J. Ciolkosz of Homer Glen, Illinois, whose journey of life gracefully concluded on October 4, 2025 at the age of 69. Philip touched countless lives with warmth and kindness, leaving behind cherished memories that will forever echo in hearts. He was loved and cherished by many people including: his spouse Pat; his children, John Ciolkosz (Kelsey), Jason (Janet) and Amy (James); his grandchildren, Braelynn, Colton, Autumn, Nathan, Waylan, Archer and Ramona; and his sister Christine (late Larry).
Visitation will be held on Sunday, October 12th 2025 from 12:00 PM to 3:00 PM at the Richard J Modell Funeral Home & Cremation Services (12641 W 143rd St, Homer Glen, IL 60491). A memorial service will be held on Sunday, October 12th 2025 at 3:00 PM at the same location.
