He is survived by: his parents, Richard Kubiak and Laura Kubiak (Engels); his daughter Madison Kubiak of Greenwood, IN; his siblings, Shelly Webster (Justin), Mark Kubiak (Trisha), Corry King and Todd Kubiak (Denise); his fiancée Jennifer Heene; and his fiancée's children, Miles Yelm of Kewanee, Kiersten Yordy of Kewanee and Paige Yordy of Kewanee. He is also survived by many nieces, nephews, aunts and uncles.

Memorials may be made to the family for a fund to be established.
