He was predeceased by: his parents, Frank Kuba and Rose Kuba (Trotter); and his brother Frank Kuba (Linda). He is survived by: his wife Catherine L. Kuba (Kolar); his children, Christina, Mary, Anthony and Frank; his grandchildren, Anthony, Maddie and Ajla; and his brother Michael Kuba. He is also survived by nieces, nephews, relatives and friends.
Unfortunately, due to the COVID-19 pandemic, visitation is limited to 10 people at a time.
