He was predeceased by : his parents, Dollie Davis and Samuel Forsythe. He is survived by : his children, Marlene Schultz (Richard) of Braidwood, Jo Ann Ferroli (Louis) of Machesney Park and Stephen Forsythe (Patricia) of Mackinaw; and his grandchildren, John Schultz (Teri), Christine Grawel (Jim), Patrick Forsythe (Nicole), Leah Ticknor (Grant) and Jane Ferroli (Jordan). He is also survived by 12 great grandchildren.

The family requests in lieu of flowers memorials be made in memory of Jerry to Mackinaw Christian Church.
