He was predeceased by: his parents, Frank Church and Nellie Church (Widman); his daughters, Ronda and Kim Church; his siblings, Floyd, Richard, Del, Ralph, Lloyd, Bob, Leonard, Franklin, Sally, Margaret and Pearl; and his siblings-in-law, Tony Leinenger, Ronie Endres, Fritz Reese, Laurabelle, Lucille, Evelyn and Hazel.
He is survived by: his former wife Patricia Marsh; his wife Pat Church; his daughters, Pam Bentley and Michelle Slack (Ray); his grandsons, Matt Bentley (Emily) and Michael Slack (Apryl); his great grandchild Braxton Slack; and his sister-in-law Mary Church. He is also survived by many nephews, nieces, and great friends.
