He was predeceased by: his parents, Betty Frazier (Newman) and Urban J. Frazier, Sr.. He is survived by: his wife Carol Frazier of Morris, Illinois; his sons, Jeff Frazier (Lisa) of Naperville and David Frazier (Cheryl) of Joliet, Illinois; his grandchildren, Evan, Ella, Katey, Tristan, Prestan and Shawn; and his brothers, Bret Frazier (Joanne) of Bourbonnais, Illinois and Bart Frazier (Kathi) of Bourbonnais, Illinois. He is also survived by five nieces and nephews.
Preferred memorials may be made as gifts in Urban's memory to the American Cancer Society.
