She was predeceased by: her spouse John W. Koontz; and her sibling Mary Haase (late Walter). She is survived by: her child Jacquelyn Johnsen of Woodridge, IL.; her grandchild Kari Criswell (Michael) of Downers Grove, IL.; and her sibling Marian DiCicco (late August) of Chicago.
