"Charlie" Zubulake (Jane) and Gay Anne Boundy (Derek); her grandchildren, Grace Marie, Hannah Joy and Joshua George Boundy; her brother Gerald "Jerry" Gay (Diane); and her nieces and nephews, Jamie Schlafke (Fred), Jerry Gay (Kaija), Julie Gay (Jeff Windschitl), Jennifer Foline (Joe), Sean Gay (Jana), Harry Zubulake, Condello Hostetler (late Mike), Mary Ellen Everhart (Les), Mary Coulter (Jeff), Peter Maduros, Maria Zubulake and Charles'Chuckie'Zubulake. She was also cherished by her kids, their spouses, grandkids and close relatives.

“What great memories. Phyllis would stop by the church office a couple of times a month to update her records of church volunteers and group members. She had an old fashioned desk in the office and I loved the hours she spent there. As she worked, she had non-stop stories and always a smile or a laugh. Phyllis, you are a light and I know it will continue to shine. I'll always remember you! John Hopkins”
