Millard (Oquawka, Illinois), born in Monmouth, Illinois, who passed away on June 15, 2018, at the age of 71, leaving to mourn family and friends. She was predeceased by : her parents, Herbert Forman and Louis Forman; and her brother David Foreman of Monmouth, IL. She is survived by : her husband Donald; and her children, Barbara Millard, Christine Millard, Donald Millard II of Oquawka, IL, Timothy Millard of Galesburg, IL and Gary Millard (Christa) of Oquawka, IL. She is also survived by many grandchildren, great-grandchildren, nieces, and nephews.
