He was predeceased by: his parents, James Ptak and Norma Ptak (Ernenputsch); and his brothers, James Ptak and Louis "Mike" Ptak. He is survived by: his children, Christopher Ptak (Brook) of Chesterfield, VA, James Ptak of Charlottesville, VA and Tasha Ptak (Rob Zamaro); his grandsons, Caleb Ptak and Josiah Zamaro; and his siblings, Diane Brackett (Al) of Washington Utah, Barbara Gorski of Granville, Mary Jo Burris of Peru, One and John Ptak (Pam) of LaSalle.
