We are sad to announce that on April 15, 2019, at the age of 67, Henry R Kooistra (Rockford, Illinois), born in Woodstock, Illinois passed away. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.

He was predeceased by: his parents, Jack Kooistra and Pauline Kooistra. He is survived by: his wife Janice Borchardt; his daughters, Nicole King (Michael) and Rachel Kooistra (Todd Koch); his grandsons, Benjamin and Jack King; and his brother Larry Kooistra (Sandra).

In lieu of flowers, the family will be establishing a memorial at Remedies Renewing Lives.
