With heavy hearts, we announce the death of Phyllis Ptak (Roselle, Illinois), who passed away on February 7, 2021. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.
She was predeceased by: her husband Ronald Sr.; her brother Jack O'Brien; and her parents, John P. O'Brien and Loretta O'Brien. She is survived by: her children, Ronald Ptak Jr., Brandon Ptak (Brian Hofacker) and Shellie Ptak (Jeremy Bockstahler); her grandchildren, Caitlyn Rae, Mackenzie, Dylan and Cassidy; and her sister Julie Moloznik (Greg).
